(STEP 1) -- Review Dr Fujimoto’s web site thoroughly
Check your computer to see if it can run the training program,
(STEP 2) -- Explore the Cogmed web site: click here:
(STEP 3) -- E-mail Dr. Fujimoto, introducing yourself with the following
information:
Your name, your child’s name and age
A few brief sentences about your child
Does your child have a formal diagnosis of ADHD?
Dr. Fujimoto prefers that the patient has already been diagnosed with ADHD. However, this is negotiable. Dr. Fujimoto will not perform diagnostic tests for ADHD.
Your interest in the program, and what you hope the working memory training will do to help your child/student.
Be sure to leave multiple contact numbers and an e-mail
(STEP 4) -- Dr. Fujimoto will contact you
You will be asked to complete an extensive Interview questionnaire

Complete the Interview questionnaire and send to Dr. Fujimoto by clicking on the "submit" button on the bottom of the form when completed.

(STEP 5) -- Dr. Fujimoto will review the information in the questionnaire .
